Description of Services:

The caregiver shall provide companionship, personal care, and assistance with daily activities such as dressing, medication management, and mobility support at the Client’s residence located at the address above.

Contract Duration and Payment:

The agreement shall commence on __________________ and end on __________________. The caregiver will be compensated with a monthly remuneration of R5,000, payable on the 5th of each month by bank transfer or other mutually agreed payment method.

Confidentiality and Responsibilities:
